Main Hospital - New Brunswick, One Robert Wood Johnson Place, New Brunswick, NJ 08901Job Summary The primary purpose of the Clinical Pharmacist Specialist position functions as a higher level, Lead Pharmacist to carry out a wide range of duties for patient care in his/her area of specialty for the facility or division.
Monitors patient drug therapy reviews and interprets all medication orders for accuracy, compatibility, interactions, appropriateness of medications and compliance.
Provides both the inpatient and outpatient environment.
The Clinical Pharmacist Specialist assists in developing and reviewing pharmacy policies and procedures.
Approximately 90 percent of this professional s time is spent administering his/her skills and knowledge in a specialized are of practice.
Essential Job Functions Administers specialized skills and care to a particular service in his/her area of specialization The BMT-Heme Clinical Pharmacist Specialist will make key recommendations and perform higher level pharmaceutical services to assist in the effective operations of the department as determined by the Department Director or Faculty Administrator The BMT-Heme Clinical Pharmacist Specialist will have out-patient and in-patient rounding duties as assigned to manage the continuity of care between the in-patient and out-patient Blood Disorders Program including:
Antimicrobial Stewardship and other Stewardship programs as they develop Pain Management Medication relevant cardiac monitoring Managing oral oncolytics and specialty pharmacy needed and discharge medications to improve LOSe, General supportive care and appropriate medication related lab monitoring Work collaboratively with health care professionals to develop institutional guidelines and make evidence-based decisions designed to improve patient care Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems Functions as a lead position in the department providing work direction to Staff Pharmacists and technical/support staff to ensure quality pharmaceutical services are delivered within the facility in accordance with FDA, Federal, State, Facility and Health System Standards The Clinical Pharmacist Specialist will be instrumental in developing and credentialing all facility staff with regards to the use of medications in his/her specialized area of practice.
For example:
total parental nutrition, oncology, psychopharmacology, etc.
Maintains appropriate records of dispensing as required by facility, state and federal laws Participates in Drug Usage Evaluation/Medications Usage Evaluation studies, participates in reporting ADR s, assures compliance with all policies and procedures including investigational drug studies Utilize the system of Continuous Quality Improvement to demonstrate and measure the effectiveness of pharmaceutical care via Quantifi Maintains appropriate records of dispensing as required by state and federal laws, especially with regard to controlled drugs Provides accurate drug information to physicians, nurses, patients and other health care personnel as needed, Counsels patients on dosage, usage, storage, side effects and indications of medications prescribed Consults with physicians to clarify inaccuracies, correct errors, and provide dosage adjustment Demonstrates responsibilities of ordering drugs and supplies and maintaining adequate but not excessive stock levels Participates in committees, research, and clinical program development and community services as required Assists in teaching program administration and design for health care staff as required Job Qualifications Bachelor's Degree or Pharm.

with relevant specialized experience including either a specialty residency (PGY2) or Board Certification in Oncology Pharmacy (BCOP).
Three (3) to five (5) years of experience as a licensed, practicing pharmacist is required.
Must be licensed and registered by the New Jersey State Board Pharmacy.
Effective oral and written communication skills.
Must be computer-literate with proficiency and working knowledge of database and reporting tools such as Microsoft Word, Excel, Access and PowerPoint.
